*As of February 1, 2012, all Cottonwood Physicians have privileges at Jordan Valley Hospital in West Jordan, and are accepting new patients at this time.

  
*Just a reminder that we are available after hours and weekends for sick and injured children.  Please call our office if you need immediate care after hours.

Please be aware!

If your child comes in for a well-child checkup and the doctor treats your child for a sick condition or procedure in addition to the well checkup, you may be billed a copay/co-insurance or deductible amount according to your insurance benefits.

Our No show/Cancellation Policy:

For well child checkups please call to cancel at least 24 hours prior to your visit, and for same day sick appointments we will need a 2 hour notice if you cannot make your appointment. This way we can fill that slot with another child who needs to be seen that day. For no-shows there will be a charge added to your account which is not billable to your insurance and will be the parent's responsibility. 

In order to avoid any unexpected patient balance owed, please know your benefits and coverage. Some vaccines may not be covered or covered at 100%, or services-for example: wart removal may fall into a different category of reimbursement with your insurance. Deductibles may come into effect depending on the services provided so please know your policy. This is important so there are no surprises after the insurance processes your claim.
 


For all new moms interested in breast feeding, we have an LPN on staff who is a certified Lactation Specialist who is happy to offer support and guidance to you and your new baby.

HOURS

We have changed our office and phone hours!! We are now available starting at 8:00 A.M. until 8:30 P.M, also during the lunch hour.  Our doors open at 8:00 A.M. as well if you need to come by the office.  A nurse is always available during these hours if you need advice or an appointment.  Please don't hesitate to call! We are here to meet your needs.

Monday - Friday: 8:00A - 8:30P
Saturday: 8:30A - 12:00P & 5:00P - 8:30P
Sunday: 5:00P - 8:30P
